This is obviously a case of the Hollywood Flu. Except for the slightly disarranged hair, Peter looks just like he always does -- fresh out of the makeup chair, and ready for his close-up.
And yes, this installment does open up disturbing lines of thought about the intimate uses of Spidey-webbing.
I was going to salaciously inquire as to what techniques MJ might use to extract Peter's web fluid against his will, but then I remembered that non-movie Spider-Man has manufactured, not biological, webbing. Oh well...
Mary Jane doesn't want to see a flu-infected Peter stupidly fight the Vulture, as such a thing would be quite cliched and whatnot.
